Clinical Pharmacist Job Openings at St. Luke's Medical Center

St. Luke's Medical Center is hiring clinical pharmacists to analyze the present medication regimen of the patients.

Other job duties:
  • Review medication history for any previous medications/herbals/home remedies/food supplements and verifies with the patient for any inconsistencies.
  • Check new medication orders against patient profiles for appropriateness of dose, dosage form and strength, time, route, frequency of administration and off label use including proper dilution and computation while ensuring that standards in medication ordering and administration are met.
  • Review for clarity and completeness of medication orders for vague, incomplete and undocumented verbal orders, and wrong review of medicines.
  • Screen for any therapeutic drug duplication, drug-drug interactions, drug-food interactions and drug incompatibilities.

  • Possesses a degree in Bachelor of Sciences in Pharmacy;
  • Registered Pharmacist of the Republic of the Philippines, with active PRC license;
  • Ability to communicate with all levels of medical and hospital staff, patients, and caregivers;
  • Good Communication skills;
